Mixed Use District Location Determination Form

This form is designed to assist “MBTA communities” with location determinations for mixed use districts

By completing the form below, EOHLC will use this information to evaluate the location of a mixed use district that may be used to contribute to a community's reasonable size requirement to achieve compliance with the requirements in Section 3A of MGL c. 40A.
