Guide for Obtaining Medical Records

In order to obtain medical records, you must complete the specific authorization form(s) for the records requested, including your contact information, the information of those in our custody who's records you seek, as well as the specific records sought.  Once completed, the form(s) should be sent to the housing facility, attention Medical Records Office. Once facility staff have obtained the appropriate signature on the authorization form(s), the requested medical records will be obtained, copied, and provided to the requester at the address provided.

The following authorization forms may be found below in the DOC’s Inmate Medical Records policy, 103 DOC 607, and are used to obtain the following types of medical records:

Authorization for the Release of Medical Records:
This is used to obtain medical records, excluding sensitive medical information as described below.  This form is to be used to obtain the results of COVID-19 testing.  This form is contained as Attachment #1 to DOC’s Inmate Medical Records policy, 103 DOC 607.

Authorization to Release Sensitive Medical Information:
This is used to obtain the sensitive medical information, including mental health records, records concerning drug or alcohol abuse, and HIV test results and/or any AIDS-related information.  This form is not to be used to obtain the results of COVID-19 testing.  This form is contained as Attachment #2 to DOC’s Inmate Medical Records policy, 103 DOC 607.
