What you need for Apply for a Certificate of Health and Free Sale & Export Statements
To apply for a certificate, you will need to:
- Complete and sign the entire application form
- Submit a separate application for each food product
- Note: Multiple flavors or varieties may be treated as one “product”
Fees for Apply for a Certificate of Health and Free Sale & Export Statements
Pay by check or money order, made payable to:
Commonwealth of Massachusetts
E-mailed and faxed applications will be held until receipt of the application fee. Mailed applications must be accompanied by the application fee.

This certificate is in accordance with Massachusetts General Law, Chapter 62C, Section 49A
Rejected applications
Certificates will not be granted if:
- The licensing status of the food processor is not acceptable
- The fee ($75 per requested certificate) is not received by the FPP
Overnight shipping
If you require overnight shipment of the certificate(s), you must include a pre-printed and pre-paid FedEx/UPS/other shipping company label and envelope. Shipping supplies are not available at the Food Protection Program.
