What you need

To be licensed as an Amusement/Challenge Course/Climbing Wall Inspector you must submit the following to this office.

  • Record of prior work experience as a third-party amusement inspector.
  • A copy of their General Liability Insurance Certificate in the amount of $1,000,000 per occurrence limit.
  • Update your photo: Massachusetts residents: you can either authorize the DOL to electronically access your MA RMV Photo, or provide a scanned copy of a 2"x2" passport photo.
    • If you are an out-of-state resident, you must submit a scanned copy of a 2"x2" passport photo.

Fees

Application processing fee is non-refundable.

When you are finished with your application, you will be redirected to our payment processor for payments. There is a convenience fee for all online transactions charged by our e-payment vendor. Online credit card transactions incur a 2.39%, debit card transactions incur a 2.09% convenience fee, or ACH/EFT (electronic checks) transactions incur a fee of $0.35.
