What you need for Apply for a license to engage in the sterilization or sanitation of bedding, upholstered furniture, and filling materials
To apply for a license, you will need to:
- Complete and sign the entire 2-page application form (found in the downloads below)
- Submit a complete copy of the sanitizing compound product label used in your process
- This should include the sanitizing compound’s technical literature and Product Safety Date Information
Please submit a separate application and fee payment for each facility and location to be licensed.
