What you need for Apply for a massage therapy license
- A sealed envelope with your official transcript from your massage therapy program.
- A 2x2 passport-style photo.
- A list of your clock hours (included in the application). If your hours are listed as credits, your school must provide a breakdown of clock hours on letterhead.
- Two signed reference letters: one from an employer in the field and one from someone unrelated.
- A copy of your insurance policy’s declaration page with the coverage date.
- Proof of passing the MBLEX or NCBTMB exam if you don’t meet the 300-hour Massage Theory and Technique requirement. Check the curriculum guidelines on the How can I become a massage therapist? page for more details.
- A CORI Acknowledgement Form.
- You must mail the Board of Registration of Massage Therapy any certificate of standing for licenses that have been held in other jurisdictions, showing the license status and disciplinary history. A certificate of standing is also known as a verification of licensure.

Once the Board gets all required documents, they’ll email you. If anything is missing, you’ll be notified by email.
After your application is processed, a license will be mailed to your address of record and your information will appear on the DOL's "Check a License" page.

Once the Board has received all of the required documents and application, an email notification will be sent to you. If you are missing any documents, an email will be sent notifying you of the information that is still required.
