The Details of Apply for Occupational School Financial Certification
What you need for Apply for Occupational School Financial Certification
For Renewal Applicants Only - You will receive a pre-notification 90 days prior to the end of your license year and will receive the application, via email, 60 days prior to your license year. The following documents and information are required for the application:
- Names, titles, and contact information for the school and relevant school leadership;
- Articles of Incorporation, Articles of Organization, or Business Certificate;
- IRS 501 (c) (3) Determination Letter if the school is a not for profit;
- Documentation of Registration with the Secretary of the Commonwealth if the school is registered with the Secretary of the Commonwealth;
- Proof of Accreditation if the school is accredited by an accreditor recognized by the U.S. Department of Education;
- Information of any pending litigation or disciplinary action (if applicable);
- Independently prepared comparative reviewed /audited financial statements for the two most recently completed fiscal years prepared on an accrual basis (refer to the Documentation Requirements for Financial Statements section of the application for details regarding the specified type of financials that would apply to your school). Please note, tax documents or internally generated statements are not accepted;
- Independently prepared comparative reviewed/audited financial statements prepared on an accrual basis for the parent entity if a parent entity exists (refer to the Documentation Requirements for Financial Statements section of the application for details regarding the specified type of financials that would apply to your school). Please note, tax documents or internally generated statements are not accepted;
- Prior year bank statement (for all school accounts) as of fiscal year-end;
- Current bank statement (for all school accounts) for the most recently completed month;
- Total Actual MA Occupational School Enrollment for the school’s prior licensing period (new starts only);
- Total Actual MA Occupational School Tuition Income for the school’s prior licensing period (new starts only);
- Total Projected MA Occupational School Enrollment (new starts only) within the school’s upcoming licensing period;
- Total Projected MA Occupational School Tuition Income for the school’s upcoming licensing period (new starts only);
- Documentation of the school’s current bond, letter of credit, or term-deposit account;
- Enrollment Agreement that includes the refund provisions of MGL Chapter 255, Section 13K;
- Tuition costs per course and payment plan info;
- Projected Course Schedule (start and end dates specified) for the school’s upcoming licensing period;
- Total actual monthly tuition income, separated by source (self-paid, employer paid, government sponsored, institutional/other private loans, Title IV Federal Student Aid, Veterans’ Administration, and institutional grants), for the current licensing period.
- Documentation of eligibility to participate in U.S. Department of Education Title IV Financial Aid (if applicable); and
- Annual Adjusted Gross Revenue (AAGR) information for the school’s most recent fiscal year period (if the school is applying to the DPL for renewal licensure).
PLEASE NOTE: APPLICATIONS ARE DUE AT LEAST 30 DAYS PRIOR TO THE END OF YOUR LICENSE YEAR

For Renewal Applicants only- an invitation to apply will be sent to the main contact email on record and will include a link to access the OSA online application. Refer to the schedule below for dates pertaining to when the invitations will be sent out and for application submission deadlines:

Pursuant to Chapter 112, Section 263 of the General Laws, the Office of the State Auditor (OSA) is required to monitor the ownership, organization, and financial condition of each entity licensed or applying to the Division of Occupational Licensure (formerly the Division of Professional Licensure (DPL)) for an original license to offer postsecondary career-oriented training programs in MA and to annually establish the level of indemnification/ surety protection needed by each school to cover potential refunds payable to students.
