Applicants must meet the following eligibility requirements:

  • Be a non-profit organization, public (municipal or state) agency, or higher education institution in Massachusetts;
  • Serve the Massachusetts Military, Veteran Family, and Survivor Community;
  • Submit all required items listed below on or before 5pm EST, Friday, January 5, 2018.

Applicants must complete the brief online application and then submit the other forms described in the steps below.

  2. Submit the Program Description and Budget

    Applicants must submit the “Required Program Description and Applicant Response Questions” and the “Budget and Budget Narrative” Form to on or before 5pm EST, Friday, January 5, 2018.

  3. Submit the Commonwealth Terms and Conditions and Authorized Signatory Forms

    Applicants must submit the “Commonwealth Terms and Conditions” and “Contractor Authorized Signatory Listing Form” with the original ink signature (photocopies/faxes/emails are not acceptable) to the address below:
