Return Assisted Living Certificates to Elder Affairs
The current Sponsor must return all Assisted Living Certificates to Elder Affairs by registered mail within 5 days after the transfer of ownership is completed.
Transfer of ownership completion notification
The new Sponsor must submit a signed and notarized statement that the transfer of ownership has been completed within 5 days after the date of closing.
