What you need for Change your name, address or request a duplicate license for cosmetology, barbering, and electrology
You can submit requests online or by mail. It's recommended to use the online option as it usually leads to faster processing.
Important points before you start:
- Shop licenses are not transferrable. Shops moving to a new location require a new application.
- A legal name change requires uploading the legal name change documents or a copy of a current government-issued ID reflecting the name change (driver’s license, passport, etc.). You must also surrender the old license to the board office at the address in the “By mail” section.
- If unable to upload the documents, mail in and attach the application ID reference number to the document. (Current Year-XXXX-AMEND) with your old license card.

How to request Change your name, address or request a duplicate license for cosmetology, barbering, and electrology
Apply online in the ePLACE portal.
First-time users can view detailed instructions for creating an account and linking your license to your account. You can request your Record ID and authorization code if you don't have them.
Once you are logged in to ePLACE and you license is linked:
- Select "Manage Licenses, Permits & Certificates"
- Select "Amendment" in the action column in the table that displays license information
- Select "DPL Amendment (name change, address change, duplicate license"
- Select the option for the appropriate request type
Complete the application form and mail it with any supporting documentation to:

- Once your application is processed, your license should arrive by mail in 2-3 weeks. You can check your license to see if your application has been processed. If it has been 6 weeks since your license was processed and you have not received it in the mail, contact the board.
- A duplicate license request will give you another physical copy of your license. The request will not send you a duplicate renewal form.
