What you need   for Chemical Addition Report

The Massachusetts Department of Environmental Protection (MassDEP) Drinking Water Program (DWP) has updated the monthly Chemical Addition Report. In accordance with 310 CMR 22.15(4) all public water systems that add chemicals to their water must submit a monthly Chemical Addition Report to MassDEP/DWP. 

The Chemical Addition Report helps to track the amount of chemical used, daily production of the water system, and the amount of chemical residual present in the distribution system. DEP/DWP reviews these reports to ensure that each public water system’s treatment is operating efficiently and effectively to provide proper protection of drinking water. The updated report is compliant with the Drinking Water Standards, and includes guidance and instructions.

Public Water Suppliers must begin using the updated form starting January 1, 2019.  You must submit your first updated form (for the month of January) by February 10, 2019.

There are three versions of the report to choose from:

  • MS Word version (C-ADD) - A blank Word form that can be filled out using MS Word or printed and completed by hand.
  • Blank, unlocked Excel version (C-ADD) - For users interested in SCADA integration. 
  • Macro-enabled, "smart" Excel version (C-ADD-XLSM) - Built-in automated calculations, i.e. formulas. This version is intended for use by small systems.  This version is not suitable for large community systems using more than a few chemicals. Minimum system requirements: Windows 7, Excel 2010.
