What you need for Establishment & duplicate certificate applications: Board of Embalming and Funeral Directing
- The Establishment Certificate Application form is to be used for new funeral establishments and for when an existing funeral establishment is changing ownership.
- The Duplicate Establishment Certificate Application form is to be completed to order a duplicate establishment certificate. Only use this form if your funeral home has been previously approved.
Incomplete applications may be returned unprocessed.
Fees for Establishment & duplicate certificate applications: Board of Embalming and Funeral Directing
Fees are non-refundable and non-transferable. Payments may be made with a check or money order. Please make checks or money orders payable to The Commonwealth of Massachusetts.
- The Establishment Certificate Application fee will be collected after the inspection process for final approval of the application.
- The Duplicate Establishment Certificate Application fee must be mailed in with the completed application.

Board inspection
Once submittal of your Establishment Certificate Application is complete, you will be contacted by a Board inspector for an inspection for compliance with the Board’s regulations (other than accessibility). Your application cannot be approved by the Board prior to these steps being complete.
Completed applications will not be approved until 4 to 6 weeks for processing after ALL required material have been received by the Board office.
