What you need   for File a health care complaint

To file a health care complaint with the Attorney General's Office, you'll need: 

  • The full name of the provider, hospital, or business you are dealing with
  • A detailed description of the problem and the steps you've already taken 
  • Due to the COVID-19 emergency, walk-ins to the AG’s physical office locations will not receive in-person service. Walk-ins will be encouraged to file complaints online or call our office.

How to file   File a health care complaint

Fill out the Attorney General's Consumer Complaint Form online by taking the following steps:

  • Select the Health and Healthcare Insurance button under "Complaint Type"
  • Enter as much information about the provider, hospital or business related to your complaint as possible
  • Describe your complaint in detail
  • Enter your name, address and any information that the Attorney General's Office can use to contact you

If you would like to submit a health care complaint by mail, please contact the Health Care Helpline at (888) 830-6277 for assistance.

Next steps   for File a health care complaint

  1. Save any documents related to your complaint

    Save all the documents related to your problem, including:

    • Bills
    • Insurance documents
    • Letters

     You may be asked to provide this information by mail or fax. 

  2. A member of the Attorney General's office will contact you.

    Your health care complaint will be carefully reviewed by Health Care Division staff. The Health Care Division offers a free mediation service, and if your complaint is appropriate for mediation, a staff member will contact you. If we determine that your complaint is not appropriate for mediation, a staff member will offer you information about other courses of action available to you.
