Renew your Assisted Living Residence Certification

You must renew certification of an Assisted Living Residence (ALR) every 2 years.
Your Application to renew your certification must be filed and date-stamped at the Executive Office of Aging & Independence (AGE) at least 30 days before the stated expiration date for your certification to no longer expire on that stated date.

The Recertification Application should be submitted via electronic file, along with all supplemental information to ALRHelp@mass.gov 

  Actions required as part of the electronic file (e- file) submission process:

  • All emailed submissions must identify the ALR name in the “Subject” line.
  • All submissions with accompanying documents must include a table of contents for each file.
  • If the size of the application is too large to be transmitted in a single email you will need to submit your application in multiple emails.
  • If multiple emails are required then the subject line should also note the number of each individual email including the total number of emails sent i.e. (1 of 3, 2 of 3 etc.).
  • If multiple emails are required to transmit the application, the required table of contents noted above must have a reference to which email batch the documents can be located. Upon the receipt of the e-file application by AGE, the Assisted Living Residence will be deemed certified in accordance with CMR 12.03(7) unless AGE notifies the Sponsor that the Application for renewal has been denied.
