What you need for Request a National Interest Waiver (NIW) Support Letter
For Massachusetts Supported Physicians:
- Provide a completed and current Application Information Sheet.
- A request letter from the heath care facility where the physician is or will be practicing.
For Non-Massachusetts Supported Physicians:
- Provide a completed and current Application Information Sheet.
- A request letter from the heath care facility where the physician is or will be practicing.
- Copy of the physician’s current resume and Massachusetts license to practice.
- Copy of the signed employment contract as evidence of a valid position.
- Facility description.
