The Board of Registration in Medicine offers an ability for all Massachusetts Physician licensees to retrieve a copy of their certificate and wallet card, and previously submitted applications, available in their user account in our License Management system.
How to view Retrieve my Physician Certificate and Wallet Card, or my previous License Applications
To retrieve your certificate and wallet card, or application, you must already have a user account and link your account to your license, and have submitted the application via this account. The video tutorials below (each less than 90 seconds in duration) will provide you with step-by-step instructions to assist you in creating an account and linking your account to your license.
After linking your account to your license, select "Manage My Licenses and Applications" and then "Show details". You will see the options to download license documents, or download previously submitted applications.
Applications submitted before October 25, 2022 are NOT available from your user account.
