What you need

Refrigeration license holders may utilize our online licensing portal to change their address/name. 

Pipefitters and Sprinkler Fitters should complete the mail-in form attached below. 

Note that it is your responsibility to keep your contact information updated, including mailing and email address. Any name change requests must be accompanied by evidence of name change (marriage certificate, court decree, etc.).

How to update

Requests can be filed in OPSI's licensing portal. Once logged in, look for the "Address Update" option in the top left to change your address or name. Online requests are only available to refrigeration technicians.
