THIS APPLICATION PROCESS HAS CHANGED. DO NOT GENERATE A TRANSMITTAL NUMBER FOR THIS APPLICATION TYPE. PLEASE REVIEW THE INSTRUCTIONS CAREFULLY AND DO NOT GENERATE A TRANSMITTAL NUMBER FOR ANY CHAPTER 91 APPLICATIONS.

A Certificate of Compliance verifies that a project licensed pursuant to Chapter 91 has been completed according to the plans, specifications, and conditions of that license. It is necessary to ensure that projects covered by Chapter 91 are constructed and used as licensed. Waterways licensees are required to file an application for a Certificate of Compliance within 60 days of completing the project, and no later than 5 years from license issuance (or any extension). A Chapter 91 license may be revoked if a certificate is not obtained.

This application is generally not required for any Chapter 91 permit, General License Certificate, Simplified License, amnesty license, or amnesty interim approval. If you have questions, please contact the Waterways Program.

  1. Review the Instructions

    The Licensee (not a representative) must complete and execute this form. A consultant or other representative may then upload it to the "Documents" section in the online application form.

  2. Complete the Application Form

  3. Submit your Application and Fee Payment

    1. File online via the EEA ePlace Portal.
    2. You will have the option to pay online by credit card or automated clearing house (ACH) or pay by mail. If you select pay-by-mail option, instructions on how to do so will be provided when you submit your online application.
