Required forms

You must include one of the following forms with your license application:

  • Department of Transportation (DOT) Medical Certificate

Most applicants submit a DOT Medical Certificate with their application. View this national registry to find a certified medical examiner to complete your DOT Medical Certificate.

  • ANSI/ASME B30.5 Medical Qualifications Form

This form is used by applicants who do not have a commercial driver's license (CDL). The ANSI/ASME Medical Qualifications Form, including drug testing, must be completed by your primary care physician (PCP). 

  • Massachusetts Intrastate Medical Waiver

Applicants with health conditions listed in 540 CMR 14.04(d), may get an Intrastate (within the borders of Massachusetts only) Waiver if their condition will not interfere with the safe operation of a commercial motor vehicle, as certified by their physician.
