OIG Academy Forms

The OIG Academy is the training program of the Office of the Inspector General (OIG).

Many of the forms for the OIG Academy (formerly the MCPPO Program) are now available online. Use these forms to register for classes, apply for the MCPPO Designation or renew your designation, get help with your Chapter 30B public procurement questions and more.

We prefer that you use the online forms that can be submitted through the website. Using the online forms is quicker, safer, and ensures a faster response.

OIG Academy Online Forms

OIG Academy Registration and Online Payment 

You can register and pay online for OIG Academy classes. Fees may apply.

OIG Academy Registration Form - Pay By Check 

You may pay for all classes for which you register with one check.

MCPPO Designation Application (First Time or Renewal) 

If you meet certain requirements, you can apply for an MCPPO Designation or Associate MCPPO Designation. Jurisdictions in Massachusetts widely recognize the MCPPO Designation as an indication that you are familiar with Massachusetts procurement practices.

Interdepartmental Encumbrance (IE) Form – For State Agencies or State Employees Only 

The interdepartmental encumbrance (IE) form may only be completed by state agencies or state employees interested in attending OIG Academy classes.

Chapter 30B Forms

Chapter 30B Technical Assistance Form 

The OIG's Chapter 30B technical assistance team can help you answer public procurement questions and can advise you on interpreting, analyzing and applying the provisions of Chapter 30B.

Chief Procurement Officer (CPO) Delegation Form 

A local jurisdiction that wishes to use Chapter 30B’s request for proposals (RFP) process to procure supplies or services ($50,000 or more) must appoint a CPO.

When you submit the MCPPO Designation application, you must also complete and submit the CORI Acknowledgement Form. The form must be notarized and include a photocopy of a government issued identification card. Incomplete CORI forms will delay application processing. The notary public will witness your signature.
