The Department of Developmental Services (DDS) provides funding for a range of Family Support Programs, including Family Support Centers, Autism Support Centers, Cultural/Linguistic-Specific Family Support Centers, Intensive Flexible Family Support Programs (IFFS), and Family Leadership Programs. These services are designed to provide an array of supports to families who have a family member with an Intellectual and Developmental Disability, or Autism Spectrum Disorder, living at home with them.

DDS has developed an on-line survey to hear from families about their experiences using Family Support services, what services and supports have been most helpful, and if there are ways we can make these services more beneficial to families. We will be re-contracting these Family Support Program services for July 2020, and your feedback and ideas will help us in the development and design of these programs and services. These survey results will supplement the input we received from the five in-person regional forums with family members we held at the end of October.

The survey is anonymous and we expect it will only take you about 10 minutes to complete. Please click on the link below. We request that you complete the survey as soon as you can, ideally by December 10th, but the survey will remain open through the month of December. In addition, we are planning to have the survey translated in several other languages (Spanish, Portuguese, Chinese, and Vietnamese) and these will be forwarded on as soon as they are available, likely within the next two weeks.

We value your input and truly appreciate you taking the time to share your feedback with us.
