Change Your Address, Direct Deposit, or W-4P Tax Information (MSRB)

MSERS Retirees & Survivors should use the forms below to change their current Address, Direct Deposit, or W-4P Federal Tax Withholding information on file with the MSRB.

To update your Address, Direct Deposit,* or W-4P Federal Tax Withholding information:

  1. Download the applicable form below
  2. Complete the form and be sure to sign and date it
  3. If submitting a Direct Deposit Form, be sure to include a voided check if being deposited into a checking account 
  4. Mail your form to:
