MFP Demonstration Services

Money Follows the Person (MFP) Demonstration

The Money Follows the Person (MFP) Demonstration provides a range of services and supports that help elders and people with disabilities move from facility-based care back to the community. Prior to moving, a transition coordinator works closely with the individual to plan and arrange services that will be needed to live safely in the community. The MFP enrollee and transition coordinator may also work collaboratively with a case manager who will help develop an individualized service plan to support the individual’s ability to remain in the community.

MFP Demonstration Services Include

Case Management

MFP provides case management for an  Demonstration Participant who does not otherwise have access to a Case Manager. The case manager can provide care coordination and assistance with obtaining necessary MFP Demonstration and State Plan Services, as well as other medical, social, or educational services, regardless of the funding source, that support the MFP Demonstration Participant’s ability to reside in a community setting. MFP Demonstration case management is available to eligible Participants during the 365 day period in the community following their transition to the community from an MFP Qualified Facility.

Assistive Technology

MFP provides assistive technology to an MFP Demonstration Participant when it is not available to the Participant under the State plan or a 1915(c) Home and Community Based Services waiver. MFP Demonstration Assistive Technology includes: devices, controls, or appliances that enable an MFP Demo Participant to: increase his or her ability to perform activities of daily living; perceive, control, or communicate with the community environment in which he or she lives; and other durable and non-durable medical equipment not available under the State plan or a HCBS waiver. 

Examples of assistive technology equipment include: 

  • communication boards; 
  • speech amplifiers; 
  • electronic device that produces speech or written output; 
  • voice-activated or remote environmental control unit; 
  • specialized door opener; 
  • customized iPad, and 
  • specialized alarms.

Orientation & Mobility

MFP provides orientation and mobility instruction that teaches an MFP Demonstration Participant with vision impairment or legal blindness how to move or travel safely and independently in his/her home and community. The instruction can include direct training, environmental evaluations, and caregiver/direct care staff training on sensitivity to blindness/low vision.

Transition Coordination

Transition Coordination

MFP provide transition coordination for an MFP Demonstration Enrollee’s transition from a facility to a community-setting. Each MFP Demonstration Enrollee has a designated Transition Coordinator. Transition Coordination functions include, but are not limited to, developing a Transition Plan, assisting the Enrollee in the HCBS Waiver application process, collaborating with the Enrollee’s Case Manager, administering an initial Quality of Life Survey and coordinating the purchase of transitional assistance services for the Enrollee.

These services may include:

  • finding accessible and affordable housing;
  • moving expenses;
  • security deposits;
  • home furnishings;
  • home modification;
  • peer support;
  • case management;
  • assistive technology devices; and
  • orientation and mobility services.
