Trade Forms (students)
Below are forms used by vendors, career centers and/or trade participants while in training
Academic Course Outline (Excel File) For all Trade approved academic programs, (Certificate and/or Degree) at any College or University the school must provide the Trade Program with an individual course outline for the client.
Participant Reimbursement Form (Word document) This form is for participants who request reimbursement for items that are required of all students in order to complete the training program.
Completion TRA Benchmarks Form (Word document) and Instructions (Word document)
All participants under the 2011 and 2015 rules are required to have benchmarks showing satisfactory performance and progress for all trainings longer than 3 months. Benchmarks are required as a part of the eligibility to receive up to 13 weeks of Completion TRA cash benefits while in training.
