Instructions on How to Request a Verification Online

Please be aware if you wish to request a verification of your license or Certificate of Regulatory Boards we do not accept paper/mailed requests. All requests must be submitted online through our e-place portal system.

Please follow the steps below to create a verification request via the online portal:

  1. Log in to your account or create an account on the E-place Portal.
  2. Click on the Manage Licenses, Permits, Certificates
  3. File an online application (accept terms)
  4. Click on License Verification Request/ Certification
  5. Select the Board for your license
    • License Type
    • Last Name
    • Contact (phone or email)
  6. List any special instructions
  7. Certificate of Regulatory Board Form requests, please upload form in the documents section. If applicable.
  8. Review information for final submission.
  9. Pay Fee

Please allow two weeks for processing. The status will change to “closed” once the verification has been completed by staff.

Change of Name, Address or Duplicate License Card

The Board is no longer accepting paper change request forms.

All applicants must complete an application on the Board’s ePlace Portal.

First time users, visit the ePlace portal to register for an account. You will need to link your license(s) to your account using your Record ID and authorization code. If you are not a first time user please skip to paragraph 4.

Once your license(s) are linked you can go to the “Manage Licenses, Permits & Certificates” tab and select the “Amendment” link under action column in your license record. Then, select <“DPL Amendment (name change, address change, duplicate license)” and hit continue. You will then need to select the option for the appropriate request type.

Important Points to be aware of before you start:
Legal name change requires uploading legal name change documentation or copy of current government issued ID reflecting the name change (driver’s license, passport, etc. and surrender of the old license to the Board office: Division of Professional Licensure, Board of Cosmetology and Barbering, 1000 Washington St. Ste 710, Boston, MA 02118.

If unable to upload the documents, mail in and attach the application ID reference number to the document. (Current Year-XXXX-AMEND) with your old license card.

Name changes require a new card to be issued, therefore there will be a fee incurred at the end of the transaction.
