VR Provider Recertification Application

Vocation rehabilitation providers must renew their certification yearly.

To apply for recertification as a vocational rehabilitation services vendor you must re-apply. A renewal letter is sent out to you from the Office of Education and Vocational Rehabilitation (OEVR), dated the first Monday in March of the current year, with attachments for you to fill out and return. All information must be received by OEVR no later than the last Friday in April. When submitting your recertification information, all providers are required to include their e-mail address on all correspondence including their resume. In addition, any company employing several vocational rehabilitation specialists are also required to indicate the specialist's e-mail address on their resume. The Department of Industrial Accidents operates an electronic document management system so all information will be sent to OEVR via e-mail.
