Eligibility
In-home vaccinations are available for anyone who has difficulty getting to or using a community vaccination location, including children.
How to schedule an in-home vaccination
By phone:
Call the In-Home Vaccination Central Intake Line at (833) 983-0485.
- CDR Health, which operates the call center, can schedule appointments and deliver in-home vaccinations Monday through Friday from 9 a.m. to 5 p.m.
- Representatives are available in both English and Spanish, and also have access to translators for over 100 other languages

Follow these four steps:
- Create an account or if you already have an account, log in.
- Select get vaccinated.
- Register for your visit.
- Present your code during your scheduled visit.
What to expect
In-home vaccinations will be:
- Scheduled within a week of request
- Performed by fully vaccinated medical professionals, following all public health safety guidelines
- Tailored to meet the individual needs of the individual, as discussed on the scheduling call
Pfizer and Moderna vaccines will be routinely offered to all individuals 6 months and older. Novavax vaccine is also available upon request when booking the appointment.
