Financial Hardship Request for MassHealth Providers

Please complete the following information and questions to be eligible for a Financial Hardship Payment:
  • All fields are required to be completed. Missing information will delay the process of your request.
  • MassHealth payment will be transmitted to your bank account via Electronic Funds Transfer (EFT) in approximately two weeks from the time your completed request is received to the time the fund is available.
  • Visit the Vendor Web Office of Comptroller and view the scheduled payment or payment history section for payment information.
