We encourage you to use the form below to make a public records request (PRR) of MOD. This helps to ensure the quickest and most accurate response to your request. Alternatively, requests may be delivered by hand, mail, email or fax.  While requests for public records may be made orally in person to a Primary Records Access Officer (RAO), such requests may not be the subject of an appeal.  Telephone requests may be accepted at the discretion of the RAO.

NOTE: MOD likely does not have records relating to an individual’s disability benefits. Instead, you may be able to get a copy of information about disability benefits from the Social Security Administration (SSA) or from the Department of Transitional Assistance (DTA). Click the links to make your request.

MOD generally does not have copies of disability determination letters. Instead, you may request these from the Massachusetts Rehabilitation Commission.
