Please have employees who are working under your Massachusetts private investigator/watchguard license complete the employee affidavit form. License holders do not need to submit themselves in the form. If you do not have any employees no action is needed on your part.
Employees will need the company's email address, license number, company name, and resident manager's name for the completion of this form.
Employees shall complete this form for each business or facility which they are employed by. No person who has been convicted of a felony or crime of moral turpitude shall be employed be a licensee.
