Switching to Paid Family and Medical Leave (PFML)

If you are switching to PFML, it is imperative that you inform the Department of Family and Medical Leave (DFML) as soon as possible.

An employer must have a registered leave administrator with DFML. The designated leave administrator must create an employer account and will be asked to enter an email address and password. It is important that the designated leave administrator:

  • Use their work email address, not a personal email address.
  • Use the same email indicated on this form so DFML can verify it.
