What you need

People born in Massachusetts will need to file the following evidence to amend a birth certificate in order to change the name of a parent or parents due to their change in sex designation. Evidence can be filed through the Registry of Vital Records & Statistics:

  • An affidavit indicating parent name change due to a change in sex designation
    • If submitted by mail, the affidavit must be notarized
  • Court-certified copy of your legal name change decree

Amendment fee $50 one-time
Certified copy of amended record (by mail) $32 per copy
Certified copy of amended record (in person) $20 per copy

Mail completed applicant affidavit and court certified copy of your legal name change decree with check or money order payable to Commonwealth of Massachusetts, to:
