What you need   for Apply for a license from the Board of Allied Health Professionals

The initial application process for allied health professions is managed by the Board of Allied Health Professions’ vendor, Professional Credential Services, Inc. (PCS). Questions regarding the application process should be directed to PCS by calling 877-887-9727 (toll free) or 615-880-4225.

All applicants are required to pay a non-refundable application fee as part of the application process. All checks and money orders must be made payable to the Commonwealth of Massachusetts. Please do NOT submit blank or unsigned checks or money orders with your license application or renewal form. All blank or unsigned checks or money orders will be returned.

Information for Graduates of Foreign Physical Therapy Programs: 

Applicants for licensure as Physical Therapists who have completed a program in a foreign jurisdiction that has not been accredited (i.e. NOT a state or territory of the United States, the District of Columbia, or the Commonwealth of Puerto Rico) must:

  • Demonstrate Proficiency of the English Language: If English is not an applicant’s first language, a passing score of 560 or higher on the Test of English as a Foreign Language (TOEFL) and a passing score of 4.5 or higher on the Test of Written English (TWE) are required for licensure. Official notice of a passing score must be provided to PCS. For more information, contact TOEFL/TWE Services, PO Box 6151, Princeton, NJ, 08541-6151 or by phone at 609-771-7100.
  • Verify Credentials: The Foreign Credentialing Commission on Physical Therapy (FCCPT) is the only credential evaluation service approved by the Board to conduct verification of foreign credentials. To apply for, applicants must:
  • Have their educational credentials evaluated and found to be equivalent by FCCPT; and
  • Provide evidence that the applicant is authorized to practice his or her specific discipline, without restriction, in the legal jurisdiction in which the post-secondary institution from which the applicant has graduated is located, or in the legal jurisdiction in which the applicant is a citizen.

Guidelines for License Applicants with Criminal Records:

All applicants for licensure by examination, reciprocity or renewal must be of “good moral character” as required by the Board of Allied Health Processions’ statutes and regulations. Applicants’ should consult the Board’s Policy Regarding Applicants/Licensees with Criminal Records prior to submitting an application.
