Apply for a license to operate amusement devices

Prior to operating an amusement or small inflatable device in Massachusetts, a license must be granted by the OPSI. Each amusement device, including inflatable devices, must be registered with the OPSI annually, and inspected according to regulations.

What you need

The following information must accompany this application:

  • A completed itinerary on a form supplied by the Office of Public Safety & Inspections (OPSI).
  • Total number of devices to be permitted.
  • An original insurance certificate with insured devices listed $1,000,000 per occurrence, $2,000,000 general aggregate minimum.
  • A completed certified inspector’s report on a form approved by the OPSI.
  • Manufacturer’s Data Report for each ride.
  • Documentation of the name of the trained and certified operators for each ride at time of application submission.
  • Name, contact information and certification documentation of the certified maintenance mechanic.
  • All manuals and bulletins, if not previously, properly submitted.
  • CORI Request Form for Company Owner. See CORI Policy Procedure here.
  • A copy of each Pressure Vessel Certificate of Inspection issued by the Massachusetts Department of Fire Services (Required).

Fees

A bank check or money order payable to the Commonwealth of Massachusetts. Processing fees are non-refundable.
