This annual license expires on June 30 regardless of date of issue.

To apply for a permit, you will need to:

  • Complete and sign the entire 4-page application form (found in the downloads below)
  • Submit all required source and finished product analyses outlined in the application
  • Submit labels for all products to be sold in Massachusetts
