The Details of Apply for a Solo or Multiple Establishment License
What you need for Apply for a Solo or Multiple Establishment License
For Solo Establishment Applications:
- A copy of a notarized Criminal Offender Record Information (CORI) Acknowledgement Form for the Establishment Owner, Establishment Operator and the Solo Massage Therapist
- Floor Plan with measurement specifications. Please note, if you are offering massage services exclusively off-site, then a floor plan is not required.
- If the Establishment has obtained the local permits, please include all local permits (examples: Business Certificate, Occupancy Permit, Zoning Permit, etc.).
For Multiple Establishment Applications:
- A copy of a notarized Criminal Offender Record Information (CORI) Acknowledgement Form for the Establishment Owner, Establishment Operator and the Solo Massage Therapist
- Floor Plan with measurement specifications. Please note, if you are offering massage services exclusively off-site, then a floor plan is not required.
- If the Establishment is owned by a corporation, the Articles of Incorporation.
- If the Establishment has obtained the local permits, please include all local permits (examples: Business Certificate, Occupancy Permit, Zoning Permit, etc.).
- Statement of Compliance Officer Form.
- Compliance Plan for Multiple Therapist Establishments (Sample)
You will be required to mail to the Board of Registration of Massage Therapy any certificates of standing for any signatory that has held a license/certificate in any jurisdiction indicating the status of the license and any relevant disciplinary information.

You can upload all of the required documents and submit your application on ePlace. If you do not already have an account, start by making an account on ePlace to begin the application process.

Application Processing times
Applications can take 4-6 weeks to process once all required items have been submitted in good order. Once an application is processed, a certificate will be mailed to your address of record. Your license information will appear on DOL's check a license page.

Once the Board has received all of the required documents and application, an email notification will be sent to you. If you are missing any documents, an email will be sent notifying you of the information that is still required.
