What you need
For Solo Establishment Applications:
- A notarized CORI Acknowledgement Form for the establishment owner, establishment operator and the solo massage therapist.
- A floor plan with measurement specifications. (unless you're offering off-site services).
- Local permits (if applicable), like a business certificate or zoning permit.
For Multiple Establishment Applications:
- A notarized CORI Acknowledgement Form for the establishment owner, establishment operator and the compliance officer.
- A floor plan with measurement specifications. (unless you're offering off-site services).
- Articles of incorporation if owned by a corporation.
- Local permits (if applicable), like a business certificate or zoning permit.
- A Statement of Compliance Officer form and a Compliance Plan (sample) for multiple therapists.

Fees
All fees are non-refundable.

Next steps
Application processing times
- It takes 4-6 weeks to process the application once all documents are submitted. Once an application is processed, a license will be mailed to your address of record. Your license information will appear on DOL's check a license page.
- If you are missing any documents, an email will be sent notifying you of the information that is still required.
