The Board is no longer accepting paper change request forms.
All applicants must complete an application on the Board’s ePlace Portal.
First time users, visit the ePlace portal to create an account. You will need to link your license(s) to your account using your Record ID and authorization code.
If you do not have your codes, please request them here.
If needed, please email the board for detailed instructions on linking your license massageboard@mass.gov
After logging in with the username that you created when you registered, go to the “Manage My Licenses and Permits” tab, Select the “Amendment” link under action column in your license record. You then will select the “Update Compliance officer” and fill out the requested information.
Important Points to be aware of before you start:
Change of compliance officer must be completed by the listed Operator of the license.
To complete the Amendment application you will need to know the license number for the proposed compliance officer and the required documents listed below. Incomplete requests will be closed without any updates.
Required Documents to Upload:
- Written Statement from establishment operator requesting to have old compliance officer replaced with new proposed compliance officer.
- Signed and Notarized CORI Acknowledgement Form from new proposed compliance officer.
- Statement by Compliance Officer Form signed by new proposed compliance officer.
