- All applicants must complete an application on the Board’s eLIPSE platform.

Important Points to be aware of before you start:
- Change of compliance officer must be completed by the listed Operator of the license.
- To complete the Amendment application you will need to know the license number for the proposed compliance officer and the required documents listed below. Incomplete requests will be closed without any updates.
Required documents to upload:
- Written Statement from establishment operator requesting to have old compliance officer replaced with new proposed compliance officer.
- Signed and Notarized CORI Acknowledgement Form from new proposed compliance officer.
- Statement by Compliance Officer Form signed by new proposed compliance officer.
