What you need

Please note: Temporary Emergency Certificates are valid for a period of six months from the date of approval by the Board and cannot be renewed. If you are assisting a current operator of your system to meet certification requirements, you must contact AMP on their website to register for the operator examination.

The following information is required to be included in your application:

  • Name, classification, address, ID#, and contact information of Public Water System.
  • Grade of certification applying for.
  • Staffing requirement information. 
  • Experience information about the operator designated to operate the system under the temporary emergency certification.
  • Criminal Offender Record Information (CORI)

Fees

Fees are non-refundable and non-transferable.

How to apply

  • Applications must be submitted online through the eLIPSE portal. You will need to select Permit and Variance Applications in the Application Type field in order for the Temporary Emergency Certification application option to appear in the Select Application field.
  • You will be asked to submit all required documents via the eLIPSE platform, including a DOL CORI form. Once submitted, you will be notified of any required next steps.
  • Payment will also be made via the eLIPSE platform.

  • Incomplete applications will be held open for 180 days from the date of application receipt to allow applicants to provide the missing information or documentation. After 180 days, the incomplete application will be considered abandoned, and applicants will need to submit a new application and payment.
