What you need

Please note: Application for Training Contact Hour courses are required for providers that do not have blanket approval from the Board.

The following information is required to be included in your application:

  • Name/Association and address of the provider offering the course.
  • Contact information of the person requesting TCHs. 
  • Program/Course title, location, and dates.
  • Course instructor information.
  • Course outline or agenda showing each topic covered and the time allotted for each topic. 
  • A copy of the instruction material showing what skills and knowledge the student will be able to demonstrate after completion of the course, including a copy of any presentations used.
  • A copy of the Certificate of Completion.
  • A copy of the attendance roster that will be used.
    • Note: The attendance roster must show: the name of the course, renewal credits issued, course id number, date and time the course was held, location of course, instructor’s name, attendees’ names, morning and afternoon sign in and each day sign in, operator certificate number if applicable, and proctor affidavit.
  • A copy of the course’s evaluation form.
  • A copy of the written policy on maintaining the course’s records.
  • A copy of the course’s requirements of satisfactory completion (performance and attendance) of the course.
  • Number of Training Contact Hours requested.

How to apply

  • Applications must be submitted online through the eLIPSE portal.
  • For instructions on managing your licenses and permits, view the DOL eLIPSE How-to-Guides.
  • Incomplete applications will be held open for 180 days from the date of application receipt to allow applicants to provide the missing information or documentation. After 180 days, the incomplete application will be considered abandoned, and applicants will need to submit a new application and payment.
