The Details
What you need
For Long Term Care (Nursing Home)
- Complete the application for Long Term Care in its entirety
- Provide a copy of the applicant's discharge papers (also known as a DD-214).
- Provide copies of all current health insurance cards (front and back).
- If applicable, provide a copy of any advance directives, such as Power of Attorney, living will, and guardianship order.
- Provide a copy of proof of income (Social Security, Aid and Attendance, VA Retirement, etc.)
- Provide proof of Massachusetts residency
For Residential
- Complete the application for residential services in its entirety.
- Provide a copy of the applicant's discharge papers (also known as a DD-214).
- Provide copies of all current health insurance cards (front and back).
- Provide copies of proof of income (Social Security, Aid and Attendance, VA Retirement, etc.)
- Provide proof of Massachusetts residency
Once you have completed the application and attached all documentation, you may mail it or bring it to the Admissions Office at the Veterans Home.
Fees
Long Term Care (Nursing Home)/ Memory Loss Care
The system of charges at the Veterans Home in Chelsea is determined as follows:
Veteran without spouse - $30.00 per day with a $300.00 personal exemption from monthly income. Income shall not include VA Aid and Attendance and/or VA Housebound, which shall be retained by the Home.
Veteran with spouse - $30.00 per day with a spousal exemption of $1,500.00 from income. Spousal income to be applied toward the $1,500.00. Income shall not include VA Aid and Attendance and/or VA Housebound, which shall be retained by the Home.
Residential
Veteran without spouse $10.00 per day with a $300.00 personal exemption from monthly income. Income shall not include VA Aid and Attendance and/or VA Housebound, which shall be retained by the Home.
Other Charges
In conjunction with room and board charges, Massachusetts General Laws allows the Veterans Home in Chelsea to apply for and receive any other income or benefits to which a veteran may be entitled.
Entitlement may derive from either federal or state statutes and/or regulations. An example of a federal benefit would be the aid and attendance benefit of $540.00 per month. Please note that the amount of any benefit may change over the course of time and any changes would be reflected in the next monthly statement. Veterans are also required to maintain adequate medical insurance, including but not limited to Medicare Part B.
Veterans are required to open a personal account with $300, and maintain a monthly balance of $300.
All bills must be paid by the tenth day of every month in which it is received.

Waiting list
Your application, if meets the admission criteria, will be placed on a bed availability waiting list.
Screening
A screening will take place sometime between application and admission. A nurse from the Home will conduct an assessment for appropriateness.
