What you need

You must turn in all of the items below with your application. If anything is missing, your fee will not be returned, and you will not be allowed to take the exam. 

  • A completed Annual Business License Application – Horse Drawn Carriage form with your correct home address and Social Security number (must be filled out in ink)..
  • The non-refundable application fee.
  • A list of all drivers with copies of their valid Certificates of Competency and current DOL CORI Request Form (Criminal Offender Record Information) reports, including the applicant.
  • Proof of insurance that covers the whole license period. Insurance must be at least $1,000,000 per incident and $2,000,000 total. It must be written on an “occurrence form” or have a bond/other proof approved by the OPSI commissioner.
  • A list of all horses with copies of their current health certificates signed by a veterinarian.
  • A list of all carriages with a color photo of each one.
  • A copy of approval from the local authority where the business plans to operate. 

Fees

All fees are non-transferable and non-refundable. Payment must be submitted in the form of check or money order, made payable to the Commonwealth of Massachusetts.

The processing fee must be the sum of application fees (application fee plus fees per carriage plus fees per horse).

Example: Application Fee ($25) + (1) Carriage Fee ($50) + (2) Horse Fee ($100) = $175 total fee.
