The Details
What you need
You must send all of the following with your application. If anything is missing or the fee is not included, your application will be delayed.
- A completed Horse Drawn Carriage Certificate of Competency Application with your correct home address and Social Security number (must be filled out in ink).
- A 2” x 2” photo, unless you are allowed to use your RMV photo.
- A clear copy of your valid driver’s license or CDL.
- DOL CORI Request Form.
- Non-refundable application fee.
Note: If someone willfully gives false information, their license or certificate can be taken away. An OPSI inspector will only endorse the application when giving the oath to the applicant.
Fees
All fees are non-transferable and non-refundable. Payment must be submitted in the form of check or money order, made payable to the Commonwealth of Massachusetts.
