The Details   of Apply for or Renew a Certificate of Clearance: S-License

What you need   for Apply for or Renew a Certificate of Clearance: S-License

  • A legible copy of a government issued identification (ex.: driver’s license) bearing the employee’s photograph and signature.
  • A completed application, including CORI Request Form, completed and signed by the applicant. (see downloads below)
  • A non-refundable check made out to the Commonwealth of Massachusetts for certificate fee.

All fees are non-refundable and non-transferrable.
