Learn about the procedure for adding or reducing certified Units in an Assisted Living Residence (ALR).
When an ALR would like to add or reduce the total number of certified Units, all materials must be submitted to EOEA within 30 days' notice.

What you need   for Change the number of certified Units in an Assisted Living Residence

  • Completed application for certification
    • Select "Other" at the top of the application
  • Letter from the ALR that explains:1
    • whether they plan to add to or reduce their total number of certified Units
    • where the Units are going to be added/reduced
    • what the timeline for completion will be 
  • Floor plans for the entire Residence.
    • These floor plans should identify the new Units (if applicable)
  • Updated Disclosure Statement with the new total number of certified Units.
  • Updated Residency Agreement with the new total number of certified Units.
  • If construction is required to change the Units, copies of relevant building permits

If the total number of certified units is being reduced, the ALR must also:

  • identify which specific units are being taken away
  • provide a written explanation of what their historical use was and what the space's future use will be

When an ALR would like to add or reduce the total number of certified Units, all materials must be submitted to EOEA within 30 days' notice.
