What you need for Change the number of certified Units in an Assisted Living Residence
- Completed application for certification
- Select "Other" at the top of the application
- Letter from the ALR that explains:1
- whether they plan to add to or reduce their total number of certified Units
- where the Units are going to be added/reduced
- what the timeline for completion will be
- Floor plans for the entire Residence.
- These floor plans should identify the new Units (if applicable)
- Updated Disclosure Statement with the new total number of certified Units.
- Updated Residency Agreement with the new total number of certified Units.
- If construction is required to change the Units, copies of relevant building permits
1 If the total number of certified units is being reduced, the ALR must also:
- identify which specific units are being taken away
- provide a written explanation of what their historical use was and what the space's future use will be
