If you have received a notice or a letter from the Department of Industrial Accidents, the DIA Board Number will be on that document.

Kindly note the 6/10/2020 Administrative Bulletin, indicates when a DIA board number should be included on all submitted Forms 105 and 113. The proper board number is required so the  submission can be assigned for processing. Should you not have the board number, or are unaware whether one exists, please follow the steps below to obtain one.

Please be advised that the DIA Board number is not the same as the claim number assigned by the workers compensation insurance company.

A request can be submitted to our Public Information Office at Info2@mass.gov or call (857) 321-7470. Please provide them with the following information to assist in the search:

  • The injured worker's name - including any alternate name/s the employee may have worked under previously
  • Date of Injury
  • Employer

    Please do not send us full Social Security Numbers (the last 4 digits are fine), Date of Birth, or any other personal identifiers.

If the Public Information Office is unable to locate an existing board number based on the information provided, one may be created for you for filing Forms 113 and Lump Sum Agreements ONLY. If this is the case, please email the Office of Claims Administration team, Meghan.Orio@mass.gov and Susan.Welby@mass.gov with the following information: 

  • Employee name (including any alternate names the employee may have worked under previously)
  • Employee address
  • Date of Injury
  • 1st day of disability/incapacity
  • 5th day of disability/incapacity
  • Employer name
  • Employer address
  • Insurer
  • Reason for requesting creation of a new board (to allow submission of Form 113 or a Lump Sum Agreement.)

Supporting documents with this email do not need to be sent to the Office of Claims Administration. The information provided above will be used to create a new board number only. Your email will be uploaded as documentation of the reason for creating a new board number. Once you receive the new board number, you will then need to submit your Form 105 or Form 113 to Dispute Resolution via email.

Please note it is the filing party's responsibility to put the DIA Board number on all forms. Forms submitted without the DIA may be returned without being acted on. The 6/10/20 Administrative Bulletin provides detailed instructions regarding how to submit Forms 105 and 113.
