Instructions

The Division of Insurance will process your request for a duplicate or replacement license for a fee of $25.00.
Please make check payable to the Commonwealth of Massachusetts.
In order to process your request, please download and fill in the Replacement License request form. Then save and / or print the form and mail it to the Division of Insurance with the fee due to:
