1. Review the Instructions

   • Linking your ePlace account to a previously obtained facility license, and then
   • Applying to renew your license, modify its terms, or close the facility.

2. Complete the Certification Statements

   Even when applying online, you will still need to complete the conventional Certification Form below and upload it as an attachment during the online filing process.

   You may fill out and save the Microsoft Word form on your computer. The PDF version has to be printed out and completed by hand.

3. Complete & Submit Your Application with Fee Payment

   1. File online via the EEA ePlace Portal.
      
      Note: If you are a first-time user, you will need to create an account first.

   2. When prompted, upload your completed and signed Certification Statements from Step 2 above as an attachment.
       
   3. Pay your fee online with a credit card and submit your application, or you may mail your fee payment to:
