2. Complete the Supporting Form

   Important Note: Even though you are applying online, you need to complete and sign the certification form below and upload it as an attachment during the online filing process.

   You may fill out and save the Microsoft Word form on your computer.

3. Submit Your Application & Fee Payment

   1. File online via the ePlace Portal.
      
      Note: If you are a first-time user, you will need to create an account first.

   2. When prompted, upload your completed and signed certification form from Step 2 above as an attachment.
   3. Pay your fee online with a credit card and submit your application, or you may mail your fee payment to:
