  2. Complete the Supporting Forms

    Important Note: Even though you are applying online, you need to complete the conventional supporting forms package below and upload it as an attachment during the online filing process.

  3. Submit Your Application & Fee Payment

    1. File online via the ePlace Portal.

    2. When prompted, upload your completed supporting forms package from Step 2 above as an attachment.
    3. Pay your fee online with a credit card and submit your application, or you may mail your fee payment to:
