What you need   for LES EA: Certification of Laboratory Renewal

Anyone (private corporation, individual, partnership or association, or other entity) who intends to conduct analytical measurements for the purposes of determining compliance with MassDEP standards where the MassDEP or the laboratory’s client requires such measurements be conducted by a certified laboratory must apply for certification from the Department.

The application may be used during the open renewal period (April 15th to May 15th)

  • To drop certification for analyte(s) for the coming fiscal year beginning on July 1st,
  • To request updates to information on file with the MassDEP for the laboratory (e.g., address, phone number, facilities, equipment, or personnel).

NOTE: Analytes may not be added during this process. To be considered for certification for additional analytes or methods, please complete an LES 03 EA (Chemistry) or LES 04 EA (Microbiology) application.
